Process
To refer a student to Kolburne, please
include the following information:
Psychosocial history
Current psychiatric evaluation
Current psychological evaluation
Current academic testing
Current IEP, if applicable
Medical history, relevant medical information and immunization history
Current progress reports and treatment plan if the student has been
or is currently in a treatment facility
Custody status
Admissions Process
- Referral material is received and reviewed by the Kolburne Admissions
Committee
- Personal interviews are scheduled either on our main campus
or within home communities of those students who are appropriate
candidates
- Timely placement decisions are made by the Admissions Committee
- Contracts, admissions forms, consents and agreements are received
- Dates of admission are scheduled
